Hi 6th years,<\/p>\n\n\n\n

If I have done my calculations correctly, you just completed your third week of rotations. Congratulations!!!<\/p>\n\n\n\n

For those who started with Psychiatry, you probably just finished that rotation and had to do the psychiatry examinations, and fill in the case report forms. <\/p>\n\n\n\n

Here<\/a>, I am uploading my forms as a SAMPLE<\/strong> to give some direction in case you are a bit lost on how to log it. <\/p>\n\n\n\n

If you are unable to find the form templates, visit the psychiatry department’s webpage here<\/a>. Go to “Download” >> “Gradual” and then scroll to the bottom of the page to access it.<\/p>\n\n\n\n

I will say, the forms did not need to be submitted during the examination, contrary to what I and many others thought. We only had to present the case during the exam (in my case, only once case was presented) and were asked questions based on it. <\/p>\n\n\n\n

All that to say, it’s not such a big deal. For the exam, I wish you the best of luck and stay tuned for the internal medicine forms. <\/p>\n\n\n\n
